Ochsner 65 Plus is designed exclusively for older patients. It offers personalized healthcare, social support, and wellness resources to care for all aspects of a person’s well-being.
Patients benefit from a multidisciplinary team that collaborates to deliver customized care plans. It includes a primary care physician who provides extended one-on-one time, with appointments often lasting between 40 minutes and one hour.
Care teams also include a licensed clinical social worker trained in behavioral therapy, a clinical pharmacist to help people manage even the most complex of medication regimens, a dietitian who can help provide nutritional counseling support, a health coach, a fitness instructor and others.
